Educational Benefits Information
Support our students by completing your Educational Benefits form
Educational Benefits forms are typically known as the annual "Free and Reduced Lunch Forms.” Did you know that by filling out this form, you help provide additional funding and resources that benefit students in other ways? These funds help to provide our students access to counselors and social workers, as well as additional reading and math support. It also helps provide necessary instructional supplies and can offset the cost of student activities fees.
We are fortunate to be able to access funding to provide free breakfast and lunch to all of our students during this pandemic. We are so thankful for this much-needed financial reprieve for families! One consequence of the free breakfast and lunch program is that many families have not completed the annual Educational Benefits form as they typically would have. Unfortunately, this means that funding for other programs is affected.
We need your help! If you haven’t filled out your Educational Benefits form yet, you can make a tremendous difference for our students by doing so. If you are not sure if you would qualify, please take a moment and fill out the form. If your family circumstances have recently changed due to loss of employment or other financial burden, you may also qualify.
The easiest way to fill out your Educational Benefits form is to do so online using your Infinite Campus Parent Portal. For assistance using the Parent Portal to fill out the form, contact the Faribault Education Center at 507-333-6472.
You can also access the Educational Benefits form using the links below. All applications are confidential!
Hard copies can be returned to your child’s school or the district office. The form can also be mailed to our food services partner at the following address: Sodexo, 925 Parshall Street, Faribault, MN 55021
You could help your child’s school receive additional educational funds and discounts AND receive nutritious school meals for your children free or at a reduced price!
Applications for these benefits, which are mailed to all households during the month of August, are required to be completed each school year in order for your children to qualify for any benefits.
If you did not receive a form or need assistance filling out the attached application, please call our office 507-333-6772 and ask for Mary Wilson.
Applications for free and reduced-priced meals are also available in each of the school offices as well as in each of the school cafeterias.
View the letters below that explain how to apply for school meal benefits and the forms to fill out, or click the link below for instructions to sign up through the parent portal.
