Insurance Committee
PURPOSE OF THE INSURANCE COMMITTEE:
The insurance committee is an advisory committee to the District Administration and the School Board. The committee reviews the operation of current insurance programs, studies and recommends changes in coverage and renewal rates, and reviews specifications prior to bid.
YOUR ROLE AS A MEMBER OF THE INSURANCE COMMITTEE:
- Represent the members of your employee group by attending monthly Insurance Committee meetings
- Communicate information regarding insurance to members of your employee group
- Collect feedback from the members of your employee group and provide information to the committee
- Inform the insurance committee of issues that may affect the work being done by the committee, in order for appropriate discussion to take place
- Provide input to district decision makers regarding changes to insurance plans and rate changes

I WAS JUST HIRED, NOW WHAT DO I DO?
Welcome to Faribault Public Schools. There are a few forms that need to be completed before the school board can officially hire new employees. (Please note: this will determine your seniority date). Once hired, you will receive electronic forms to complete. After completing the forms you will be instructed to bring in the following items:
- Picture ID
- Social Security Card
- Voided check or savings slip for direct deposit
Our office hours are: 7:30am-4pm, Monday-Friday.
Direct Deposit/Getting Paid
DO I HAVE TO GO ON DIRECT DEPOSIT?
Yes, Your paycheck is deposited directly into your bank account on payday.
WHEN WILL I GET PAID?
Checks will be deposited on the 15th or last business day before the 15th of the month, and the last business day of the month. Less than 12-month employees: If employed less than 12 months per year, employees do not qualify for unemployment compensation during the summer months, by Minnesota Statute
Time Off Request
IF I AM GONE FROM WORK FOR A DAY, WHAT DO I DO?
Fill out the appropriate form, or submit an online request via Aesop. Once approved by your supervisor, the request is sent to the Business Office to be recorded. Time off can be viewed on Aesop and logging in with ID and password provided by Human Resource Office.

I HAVE TO TRAVEL BETWEEN BUILDINGS EACH DAY. HOW WILL I BE REIMBURSED FOR THIS?
Contact your supervisor regarding your options: • $1.12 per round trip. This is added to your payroll check after the completed and signed voucher request form is submitted to the Payroll Office. • $.535 per mile during 2020. This will be an accounts payable check issued after the completed, signed and approved voucher request form is submitted to accounts payable.
Leave of Absence
HOW DO I APPLY FOR A LEAVE OF ABSENCE? WHO DO I TALK TO? WHAT DO I DO WHEN I COME BACK FROM MY LEAVE?
As soon as you know you are going to need a leave of absence, inform your building administrator so an appropriate substitute can be hired in a timely manner. Next, submit a letter to the School Board requesting a leave with the dates you will be gone via the Human Resource Office. Include a doctor’s statement and completed FMLA Form. If you are taking a leave different from FML, see your contract to determine what steps must be made. When you are ready to return from a leave contact your building administrator, Personnel, and Benefits.

Employees of Faribault Public Schools are eligible for family and medical leave if they have at least 12 months of service with the District and have worked at least 1,250 hours within the preceding 12 month period. The following information is provided to enable you to formally apply for family or medical leave under the provisions of the Family and Medical Leave Act.
